BOSSERT, WILLIAM H., AND WILLIAM B. SCHWARTZ. Relation of pressure and flow to control of sodium reabsorption in the proximal tubule. Am. J. Physiol. 2 13(s) : 793-802. I9670 -It is the purpose of this paper to present a theoretical analysis of the interrelationship between pressure and flow in the proximal tubule and to examine its implications for the control of proximal sodium reabsorption. A model has been derived which integrates these hydrodynamic factors, and the behavior of the model has been examined over a wide range of reduced glomerular filtration rates, utilizing an automatic computer. The results suggest that during reductions in glomerular filtration rate sodium reabsorption in the proximal tubule is controlled by a negative feedback system which can account for the continued delivery of sodium and water into the distal nephron. It is noteworthy that the model behaves in a stable fashion despite considerable variations in the particular physical characteristics of individual nephrons (e.g., distensibility, length of loop of Henle) ; this stability can also be attributed to the negative feedback characteristics of the system. The model, beyond yielding certain insights into the nature of the control of proximal sodium reabsorption, also provides a framework for the incorporation of additional factors which may influence the reabsorption of sodium and water.

Assessment of proximal tubular sodium reabsorption during water diuresis in patients with heart disease.

In order to examine the relation between cardiac state and the capacity to excrete a water load, 10 normal subjects and 61 patients with heart disease were studied during water diuresis. Under these conditions, urine flow approximates to the rate of delivery of filtrate, and therefore of sodium, from the proximal tubule of the kidney to the loop of Henle, while free water clearance is a functio...

Effect of parathyroid hormone secretion on sodium reabsorption by the proximal tubule.

To determine if an increase in the endogenous secretion of parathyroid hormone could decrease sodium reabsorption by the proximal tubule, the ionized calcium concentration of blood perfusing the parathyroid gland of eight unilaterally thyroid parathyroidectomized dogs (TPTX) was reduced by infusion of an isotonic sodium citrate plus sodium chloride solution into the blood supply of the parathyr...

Proximal sodium reabsorption: An independent determinant of blood pressure response to salt.

The purpose of this study was to evaluate the contribution of renal sodium handling by the proximal tubule as an independent determinant of blood pressure responsiveness to salt in hypertension. We measured blood pressure (BP), renal hemodynamics, and segmental renal sodium handling (with lithium used as a marker of proximal sodium reabsorption) in 38 hypertensive patients and 27 normotensive s...

Effect of thoracic cava obstruction on response of proximal tubule sodium reabsorption to saline infusion.

We have recently demonstrated that in the normal dog infusion of isotonic or hypertonic saline results in marked depression of sodium reabsorption in the proximal tubule (1). This depression is independent of the effect of saline infusion on filtration rate and is not blocked by reduction of filtration rate. Such depression in proximal tubule reabsorption appears to account for the diuresis kno...

An inhibitory effect of furosemide on sodium reabsorption by the proximal tubule of the rat nephron.

The evidence from previous micropuncture studies for an inhibitory effect of furosemide on proximal sodium reabsorption in the rat has been conflicting. Intrinsic reabsorptive capacity, estimated in free flow and shrinking drop experiments, has been reported to be depressed, whereas fractional reabsorption usually remains unchanged. We have recently reported that, during conditions of elevated ...
